On the order form, please check the box next to any products you wish to order. Also be sure to enter the quantity of each item you want to order, especially in the the case of CD Copying/Manufacturing (Item #16) and the line total.

NOTE: If your order is for the purchase of an item that has to be shipped to you you MUST include the SHIPPING and HANDLING CHARGE or your order will not be sent! Funds are payable in U.S. DOLLARS only! Your order will be processed and shipped to you within one business day.

For REGISTRANTS ONLY: In your mailer be sure to include your BIOGRAPHY (100 words or less on hard copy or disk (.txt .doc .wps)), a PICTURE of yourself on disk (.jpg, .gif) or actual photo, and your PERFORMANCE VIDEO (.mpg file format only) or SONG on CD that you will use for your performance.
